fa تعیین روند میلینه شدن مغز در شیرخواران نرمال ایرانی بین دو ماهگی تا دو سالگی بین سال‌های 1400-1398 Assessment of Myelination Milestones on Brain MRI in Patients Aged 2-24 Months with Nonspecific Symptoms from 2019 to 2021 رادیولوژی Radiology پژوهشی Original سابقه و هدف: میلینه شدن ماده سفید مغز در تکامل نورولوژیک مؤثر بوده و به آسانی با MRI  قابل ارزیابی است. این مطالعه برای نخستین بار در ایران انجام شده است و الگوی میلینه شدن ماده سفید را در کودکان ایرانی بررسی کرده و مقایسه با جدول‌های استاندارد جهانی انجام داده است. روش کار: این تحقیق به روش مطالعه داده‌های موجود انجام شد که 249 کودک کمتر از 24 ماه که از نظر تکاملی نرمال بوده‌اند و به دلایل دیگری MRI مغز انجام داده‌اند را بررسی کرده است. نتایج MRI در سکانس‌های FLAIR , T2 , T1 به طور کیفی و بر اساس تغییرهای سیگنال ثبت شدند و در هر سن با جدول استاندارد مقایسه شدند. در هر گروه بیش از 95درصد کودکان الگوی مشابه با جدول داشتند، مقادیر به صورت همخوانی با جدول استاندارد برای آن گروه سنی در نظر گرفته شد. یافته‌ها: روند میلینه شدن در سنین 2، 4، 8، 18 و 24 ماهگی با جدول استاندارد در سکانس‌های  T2 , T1 مطابقت داشت، اما در سنین 6 و 12 ماهگی تطابقی نداشت و در مورد سکانس FLAIR در همه  سنین به غیر از 24 ماهگی تطابق وجود نداشت. نتیجه‌گیری: با توجه به مطابقت با جدول استاندارد در بیشتر موارد، به نظر می‌رسد که اختلاف‌های دموگرافیک کمتر مؤثر باشد؛ ولی به نظر می‌رسد که آماده کردن یک جدول نرمال با تعداد نمونه بالاتر بر اساس کودکان ایرانی منطقی‌تر باشد. Background and Aim: White matter myelination is a critical process in neurological development and maturation, which can be reliably assessed through brain MRI. This study aimed to evaluate the pattern of brain myelination in Iranian infants and compare it with global standard references- marking the first such investigation conducted in Iran. Methods: This retrospective study analyzed existing data from 249 children under 24 months of age who underwent brain MRI for various clinical reasons but were deemed neurologically normal. Qualitative assessment of myelination was performed using T1- and T2-weighted images and FLAIR sequences. Myelination stages were compared across age groups using international standard references. For each age group, if more than 95% of cases exhibited patterns consistent with the global standards, the results were considered aligned with normal developmental milestones. Results: Signal intensity changes on T1- and T2-weighted images corresponding to key myelination landmarks were documented across different age groups. At 2, 4, 8, 18, and 24 months of age, the myelination patterns on T1- and T2-weighted images were largely consistent with global standards. However, discrepancies were noted in other age groups. Additionally, FLAIR sequences showed inconsistent findings across nearly all age groups, except for children at 24 months. Conclusion: The findings suggest that demographic factors such as ethnicity may not significantly influence the timeline of myelination.
