fa بررسی تغییرات حجم جراحی‌های الکتیو کودکان پیش و پس از شروع همه‌گیری کووید-۱۹: تحلیل سری‌های زمانی در بیمارستان کودکان مفید، تهران Changes in Pediatric Elective Surgery Volume before and after the COVID-19 Pandemic Onset: A Time- Series Analysis at Mofid Children's Hospital, Tehran جراحی (عمومی، اطفال، عروق، توراکس، ترمیمی، فک و صورت ) Surgery پژوهشی Original <div style="text-align: justify;">سابقه و هدف: همه‌گیری کووید-۱۹ اختلالات گسترده‌ای در نظام سلامت ایجاد کرد و ارائه جراحی‌های الکتیو را تحت تأثیر قرار داد. با توجه به اهمیت مداخلات جراحی به‌موقع در کودکان و کمبود شواهد از کشورهای در حال توسعه، این مطالعه با هدف بررسی تغییرات فراوانی جراحی‌های الکتیو کودکان در بیمارستان مفید تهران طی همه‌گیری انجام شد. روش کار: این مطالعه تحلیل سری‌های زمانی گذشته‌نگر بر روی داده‌های ثانویه بیمارستانی بود. اطلاعات کلیه جراحی‌های الکتیو کودکان در ۱۲ بخش تخصصی بیمارستان مفید تهران از فروردین ۱۳۹۸ تا اسفند ۱۳۹۹ استخراج گردید. پس از پالایش، ۳۸۴۸ جراحی واجد شرایط باقی ماندند. تغییرات فراوانی جراحی‌ها با استفاده از مدل‌های ITSA، STSA و BSTS بررسی شد. تحلیل آماری با  آزمون‌های تی، کای دو و آزمون دقیق فیشر انجام شد. یافته‌ها: میانگین سنی بیماران پس از همه‌گیری کاهش معناداری داشت (0/041=P). به‌جز بخش‌های عفونی و گوارش، سایر بخش‌ها با کاهش چشمگیر جراحی‌ها مواجه شدند. کاهش معنادار در گروه کودکان (0/019=P) و کل جمعیت (0/001>P) مشاهده شد، اما در نوزادان معنی‌دار نبود (0/450=P). مدل BSTS نشان داد میانگین هفتگی جراحی‌ها ۲۹ درصد کمتر از پیش‌بینی بود (کاهش تجمعی ۶۴۵ جراحی؛ 0/001=P). بیشترین کاهش در بخش‌های گوش، حلق و بینی (۶۶ درصد) و ارتوپدی (۴۵ درصد) ثبت شد. زمان بازیابی در برخی بخش‌ها بین ۵۵ تا ۹۵ هفته بود، اما در کودکان و دندانپزشکی به سطح پیش از پاندمی بازنگشته است. نتیجه‌گیری: همه‌گیری کووید-۱۹ با کاهش قابل‌توجهی در جراحی‌های الکتیو کودکان همراه بود، به‌ویژه در گروه سنی بالای یک سال و بخش‌های ارتوپدی و گوش، حلق و بینی. بازگشت جراحی‌ها در بیشتر بخش‌ها کند بود و در برخی تا پایان مطالعه محقق نشد. این یافته‌ها ضرورت تدوین راهبردهای مؤثر برای مدیریت عقب‌ماندگی جراحی‌ها و افزایش تاب‌آوری نظام سلامت را آشکار می‌سازد.</div> <div style="text-align: justify;">Background and Aim: The COVID-19 pandemic caused widespread disruptions in healthcare systems and affected the provision of elective surgeries. Given the importance of timely surgical interventions in children and the scarcity of evidence from developing countries, this study aimed to investigate changes in the frequency of pediatric elective surgeries at Mofid Children's Hospital in Tehran during the pandemic. Methods: This was a retrospective time-series analysis of secondary hospital data. Data on all pediatric elective surgeries performed across 12 specialized departments at Mofid Children's Hospital, Tehran, were extracted from April 2019 to March 2021. After data cleaning, 3,848 eligible surgeries remained for analysis. Changes in surgery frequency were assessed using ITSA, STSA, and BSTS models. Statistical analyses were performed using t-tests, chi-square tests, and Fisher's exact test. Results: The mean age of patients decreased significantly after the pandemic (P = 0.041). Except for the infectious diseases and gastroenterology departments, most departments experienced a marked decline in surgical volume. A significant reduction was observed among children (P = 0.019) and in the total population (P < 0.001), but not among neonates (P = 0.450). The BSTS model showed that the weekly average number of surgeries was 29% lower than predicted, corresponding to a cumulative reduction of 645 surgeries (P < 0.001). The greatest reductions were observed in the ENT (66%) and orthopedic (45%) departments. Recovery times in some departments ranged from 55 to 95 weeks; however, surgical volume among children and in dentistry had not returned to pre-pandemic levels by the end of the study period. Conclusion: The COVID-19 pandemic was associated with a significant reduction in pediatric elective surgery volume, particularly among children over one year of age and in the orthopedic and ENT departments. Recovery of surgical volume was slow in most departments and had not been achieved in some by the end of the study period.
