Background and Aim: Malignant otitis externa is a severe and potentially life-threatening infection affecting the external ear canal and temporal bone. Prompt diagnosis is critical, as delays can lead to serious complications or even fatal outcomes. Management strategies for this condition vary and can often be contradictory and challenging to implement. Therefore, this study aims to review the clinical and radiological characteristics associated with malignant otitis externa.
Methods: In this review, the keywords “clinical,” “radiological,” “malignant,” and “otitis externa” were systematically searched in the PUBMED, EMBASE, and ELSEVIER databases for studies published from 2015 to 2025. The review evaluated clinical and radiological characteristics of malignant otitis externa, including clinical trials, cross-sectional studies, case-control studies, and cohort studies. Exclusion criteria included review articles, systematic reviews, meta-analyses, case reports, and studies focusing on treatment.
Conclusion: Malignant otitis externa and its complications are more common in diabetic patients and the elderly. Pain and ear discharge are the most common symptoms, and their presence—especially in high-risk individuals—should prompt evaluation for malignant otitis externa. CT scans and MRI are valuable diagnostic tools for this disease.
