<div style="text-align: justify;"><span style="font-size:16px;"><span style="font-family:b nazanin;">تا پانزده سال پیش پیوند مغز استخوان جنبه تجربی داشت ولی در سالهای اخیر، این مسئله در عالم پزشکی جایگاه ویژه ای پیدا کرده و امروزه به طور قطع باعث درمان بسیاری از بیماریها می باشد.</span><br>
<span style="font-family:b nazanin;">در اواخر سال ۱۹۵۵ - یعنی زمانی که پیوند مغز استخوان برای نخستین بار در انسان به اجرا در آمد - آگاهی از پادگنهای پیوند اندک بود، و بکارگیری روشهای مراقبتی و محافظتی، از قبیل فرآورده های خون مراحل اولیه خود را طی می کرد و تعداد آنتی بیوتیکهای موثر بسیار محدود بود. با همه این دشواریها پژوهندگان آمریکای شمالی و اروپا برای پذیرش پیوند و ایجاد مقاومت تلاشهای فراوانی به عمل آوردند. مطالعات گسترده در حیوانات سبب شد که چند پیوند موفقیت آمیز در مورد دهنده و گیرنده در دوقلو ها به دست آید و با کشف و آگاهی از مسائل ایمنی و </span><span dir="LTR">HLA</span><span style="font-family:b nazanin;"> موفقیت این روش رو به فزونی نهاد.</span><br>
<span style="font-family:b nazanin;">اکنون مراکز متعددی در آمریکا، اروپا و آسیا قادرند این نحوه درمان را با اطمینان بیشتر و کارآیی بالاتر در بیماران انجام دهند. لزوم پیوند در هر یک از بیماران باید به طور اختصاصی و جداگانه بررسی و اجرا شود.</span><br>
<span style="font-family:b nazanin;">نتیجه، فایده، پیش آگهی، عوارض، نحوه و خرج درمان دقیقا و به طور کامل باید به بیمار، دهنده و خانواده بیمار توضیح داده شود</span><span style="font-family:b nazanin;"> در ۱۵ سال اخیر در روش و نحوه نتیجه پیوند مغز استخوان پیشرفتهای بسیار زیادی حاصل شده است. امروزه نزدیک به ۷۰۰ پیوند از دهنده های غیر خویشاوند، در بیماران مختلف و در نقاط متعدد دنیا صورت گرفته است (۱۹۹۰,</span><span dir="LTR">ASH</span><span style="font-family:b nazanin;">) .چنین پیشرفتی بی شک در سالهای آینده با تشکیل مراکز دیگر پیوند مغز استخوان و بکارگیری برنامه های بهتر ادامه خواهد یافت و نتایج بهتری به دست خواهد آمد. به رغم مشکلات زیاد، عوارض متعدد و هزینه گزاف در حال حاضر پیوند مغز استخوان تنها وسیله درمان قطعی بعضی از بیماریهای خون می باشد.</span></span></div>

<span dir="LTR">SUMMARY</span><br>
<span dir="LTR">Up to 15 years ago, bone marrow transplantation was experimental, but in the recent years it has gained an important role in the field of Medicine, and it has clearly been established as a therapeutic modality for a wide range of diseases.</span> <span dir="LTR">During 1950-1955 when marrow transplant was first attempted in human, little was known about transplant antigens. Supportive care techniques such as blood products, effective new antibiotics were in their primitive state and quite limited. Despite these problems, investigators in North America and Europe made enormous efforts to induce tolerance and graft acceptance. Extensive studies were done in animals. Dr. E. Donall Thomas and other researchers made extensive experiments in canine and other lab animals for many years. Except for few successful transplant between identical twin donor - recipient, results of the early trials were uniformly disappointing, until the discovery of HLA antigen with more knowledge in immunohematology, then progress started to be made.</span><br>
<span dir="LTR">There are now a number of centers in the USA, Europe and in the Asia with the capability of carrying out bone marrow transplants with good Standard.</span><br>
<span dir="LTR">Each patient must be evaluated individually and the patient, donor, the family must be fully informed about the benefits, costs and expected complications of this procedure.</span><br>
<span dir="LTR">In the last 15 years exellent progress has been made in the results and procedures of the bone marrow transplantation. Near to 700 unrelated donors for bone marrow transplantation were reported worldwide in ASH Meeting held in Boston December 1990. This progress will undoubtedly continue as more and compare results.</span><br>
<span dir="LTR">Despite of the hardships, risks, costs, complications, marrow transplantation still offers the only potential cure for many patients with hematologic disorders.</span>
