fa بررسی میزان شیوع کم‌کاری تیروئید در معلولین ذهنی آسایشگاههای تهران بین رشته ای ( مدیریت آموزشی، تحقیقات آموزشی، آموزش پزشکی ) Interdisciplinary (Educational Management, Educational research, Statistics, Medical education عمومی General <p style="DIRECTION: rtl">سابقه و هدف: شیوع کم‌کاری تیروئید و اختلالات رشد ذهنی در مناطق کمبود ید افزایش نشان می‌دهد. از آنجا که شیوع کم‌کاری تیروئید در افراد مبتلا به عقب‌ماندگی ذهنی در ایران مشخص نیست، این بررسی در 2003 نفر ساکنین آسایشگاه معلولین ذهنی تهران انجام شد. </p><p style="DIRECTION: rtl">مواد و روشها: پس از تکمیل پرسشنامه برای هر فرد، معاینه بالینی و نمونه خون جهت اندازه‌گیری 4T و 3T و TSH اخذ شد. غلظت هورمونها با روش رادیوایمونواسی توسط کیتهای تجارتی اندازه‌گیری شد. </p><p style="DIRECTION: rtl">یافته‌ها: 295 نفر (17%) دارای گواتر درجه Ib، 136 نفر (7/6%) گواتر درجه II و 16 نفر (8/0%) گواتر درجه III بودند. غلظت 4T بین g/dl 5/4-1/4 در 5/1%، 4-1/3 در 7/1%، 3-1/2 در 6/0% و کمتر از 1/2 در 1/0% معلولین ذهنی دیده شد. 53 نفر (65/2%) دارای TSH بیش از mU/I 5بودند که اکثریت (15/2%) آنها TSH بین 5 تا 10 داشتند. در 10 نفر TSH بیشتر از mU/I10 بود و سه نفر نیز با تشخیص قبلی کم‌کاری تیروئید لووتیروکسین مصرف می‌کردند. در کل شیوع افزایش TSH بالاتر از mU/I5، 79/2% و افزایش TSH بالاتر از mU/I10، 64/0% بود. 76 نفر (8/3%) از معلولین مبتلا به مونگولیسم بودند که 7 نفر (9/7%) آنها کم‌کاری تیروئید داشتند. </p><p style="DIRECTION: rtl">نتیجه‌گیری و توصیه‌ها: این بررسی شیوع نسبتاَ بالای کم‌کاری تیروئید در معلولین ذهنی را نشان می‌دهد. علاوه بر کنترل اختلالات ناشی از کمبود ید، غربالگری کم‌کاری تیروئید برای کاهش معلولین ذهنی ضروری است. </p> <p><strong>Background: </strong>Hypothyroidism and associated mental disorders are prevalent in iodine deficient regions. Since the prevalence of hypothyroidism is unknown in mentally retarded  patients in Iran the present study was carried out over 2003 patients hospitalized in Tehran </p><p> institutes. </p><p><strong> Materials and methods: </strong>Following the physical examination and filling the questionnaire,  blood samples were obtained for measuring T₄, T₃, and TSH. Radioimmunoassay technique was used for this purpose. </p><p><strong> Results: </strong>Among 2003 candidates, 295(17%) had goiter of grade lb, whereas grade II and grade III goiter were reported in 136(6.7%) and 16(0.8%) of the cases, respectively. T₄ level  was reported as below: 4.1- 4.5 ug/dl in 1.5%, 3.1- 4 in 1.7%, 2.1- 3 in 0.6%, and below 2.1u.g/dl in 0.1% of the mentally retarded patients. 53(2.65%) had TSH level of more than  5mU/l, where most of them (2.15%) had TSH level of 5-10 <strong>mU/1. </strong>TSH>10mU/l was recorded  in 10 cases. 3 patients were received levothyroxine due to previously diagnosed  hypothyroidism. 76(3.8%) cases were found to have mongolism, where 7(7.9%) of them had hypothyroidism. </p><p><strong> Conclusion: </strong>Results have revealed the relatively high prevalence of hypothyroidism in mentally retarded patients.
