en بررسی همبستگی شاخص‌های لنف نود با چاقی در ماموگرافی Correlation between lymph node indicators and obesity on mammography رادیولوژی Radiology پژوهشی Original زمینه و هدف: با توجه به شیوع بالای مشاهده لنف نودهای آگزیلاری در ماموگرافی‌های انجام شده و همچنین اهمیت ابعاد هیلوم و کورتکس این لنف نودها در شک به پاتولوژیک بودن آن‌ها و نیز نقش چاقی به عنوان یک ریسک فاکتور در کنسر پستان و پروگنوز بیماران مبتلا به کنسر پستان، تعیین رابطه بین چاقی (body mass index یا BMI) و ابعاد لنف نودهای اگزیلاری می‌تواند رویکرد به اهمیت ابعاد لنف نودها را تحت تاثیر قرار دهد. این مطالعه به تعیین همبستگی بین چاقی و ابعاد لنف نودهای آگزیلاری در ماموگرافی‌های غربال‌گری می‌پردازد.  مواد و روش‌ها: ما در یک مطالعه توصیفی (از مهرماه 96 تا مهرماه 97) تعداد 204 بیمار را که برای انجام ماموگرافی غربال گری به مرکز آموزشی، پژوهشی و درمانی مهدیه مراجعه کردند، بررسی کردیم. طول و عرض لنف نودهای آگزیلاری مشاهده شده، اندازه گیری شد. معیار HCR یا hilo-cortical ratio (نسبت عرض هیلوم به عرض کورتکس لنف نود) محاسبه شد.  با استفاده از independent sample t-test و Fisher’s exact test، همراهی بین ابعاد لنف نود و هیلوم، دانسیته برست و سن با BMI بررسی شد. Multivariable linear regression برای ارزیابی همراهی مستقل ابعاد لنف نود با BMI پس از انجام adjustment با سن و دانسیته برست استفاده شد. برای بررسی inter-rater reliability بین دو مفسر ماموگرافی نیز از Pearson correlation coefficient استفاده شد. یافته‌ها: یک همبستگی قوی و معنادار بین BMI و طول لنف نود R2=0.626))، عرض لنف نود                                   R2=0.508))، طول هیلوم (R2=0.558)، عرض هیلوم  64/.=£    و     HCR 35/.=£    مشاهده شد  که همه آن‌ها مستقل از سن و دانسیته پستان است. ارتباطی بین افزایش BMI و ضخامت کورتکس لنف نود مشاهده نشد. همبستگی قوی بین افزایش در  طول هلیوم و افزایش در طول لنف نود وجود دارد ( (R2=0/95. به طور مشابه نیز ارتباط قوی بین افزایش در عرض هلیوم  و عرض لنف نود مشهود است.  همچنین همبستگی معنادار بین HCR و BMI مشاهده شد (R2=0/34). نتیجه‌گیری: به نظر می‌رسد افزایش ابعاد لنف نود ارتباطی به ضخامت کورتکس ندارد. به احتمال نودهای حاوی چربی در ماموگرافی‌های غربالگری می‌توانند به عنوان یک بیوماکر چاقی و پاتولوژیهای مرتبط به آن استفاده شود. بین ابعاد لنف نود و چاقی همبستگی عالی وجود دارد. Background and Aim: Due to the high prevalence of axillary nodules in mammograms, and the importance of hyloum and cortex dimensions  in suspicion to be pathological as well as the role of obesity as a risk factor in breast cancer and prognosis of patients with breast canister, determining the relationship between body mass index (BMI) and the size of the lymph nodes in the exile can affect the approach to the importance of the size of the lymph nodes. The current study was carried out to investigate the correlation between obesity and the size of lymph nodes in angiography in screening mammograms. Materials and Methods: In a descriptive study (from October 1996 to September 1997), we examined 204 patients who referred to Mahdieh Educational, Research, and Medical Center for mammography screening. The length and width of the observed axillary nodules were measured. The HCR criterion (hilo-cortical ratio) was calculated. Using independent sample t-test and Fisher's exact test, the association between lymph node and hyloma dimensions, Brest, and Sen density with BMI was investigated. Multivariable linear regression was used to assess the independent association of lymph node dimensions with BMI after adjusting age and brest density. Pearson correlation coefficient was also used to evaluate the inter-rater reliability between the two mammographic commentators. Results: A strong and significant correlation was observed between BMI and length of lymph node R2 = 0.626), lymph node width R2 = 0.508), helium length (R2 = 0.558), helium width 64 /.= £ and HCR 35 /.= £ all of which are independent of breast age and density. No association was observed between BMI increase and Cortex lymph node thickness. Also, a strong correlation was observed between the increase in helium length and the increase in lymph node length ((R2 = 0.95). Similarly, a strong relationship was found between increase in helium width and lymph node width. Moreover, a significant correlation was found between HCR and BMI (R2 = 0/34). Conclusion: It seems that the increase in the size of the lymph node is not related to the thickness of the cortex.
