fa روش‌های تشخیص آزمایشگاهی عفونت مالاریا Laboratory Diagnosis Methods of Malaria Infection میکروب‌شناسی Microbiology مروری Review <div style="text-align: justify;">سابقه و هدف: برای مدیریت و کنترل بیماری مالاریا - یکی از مهم‌ترین بیماریهای عفونی مناطق گرمسیری و نیمه گرمسیری - تشخیص سریع و دقیق این بیماری ضروری و از اولویت بالایی برخوردار است. به این منظور روشهای متعددی برای تشخیص مالاریا توسعه یافته است و در حال حاضر از روش تشخیص بررسی لام خون محیطی با میکروسکوپ نوری به عنوان روش استاندارد طلایی و روش‌های تشخیص سریع در مناطق آندمیک مالاریا استفاده می‌شود. هدف این مقاله بررسی روش‌های موجود و روش‌های در حال توسعه به عنوان گزینه‌های احتمالی آینده برای بهبود تشخیص مالاریاست. روش کار: برای نگارش این مقاله مروری با جستوجو کلید واژگان از جمله تشخیص مالاریا، روش میکروسکوپی، روشهای مولکولی، RDT، Nested- PCR و LAMP در پایگاه دادههای PubMed و Google Scholar، مقالات انگلیسی زبان بدون محدودیت تاریخ بررسی شد. مقاله‌هایی که بیشترین ارتباط را با اهداف نگارش این مقاله داشتند، انتخاب و مطالعه شدند. یافته‌ها و نتیجه‌گیری: بررسی لام‌خون محیطی با میکروسکوپ نوری، با وجود محدودیتهایی که دارد، همچنان روش استاندارد طلایی برای تشخیص مالاریا محسوب میشود. از دیگر روشهای تشخیصی مالاریا میتوان به آزمایش‌های تشخیصی سریع اشاره کرد که با توجه به ویژگیهای آن، جایگزینی سریع و ارزان برای تشخیص مالاریا در مناطق آندمیک و دورافتاده است. یکی از موانع برنامه حذف مالاریا، تشخیص عفونت در بیماران بدون علامت و یا با آلودگی کم است؛ زیرا عموماً با روشهای روتین مانند بررسی لام خون محیطی و آزمایش‌های تشخیصی سریع، شناسایی نمیشوند. از این جهت روشهای مولکولی در مناطق آندمیک مالاریا با برنامه حذف، مناسب بهرهگیری هستند. روشهای نوین که در تحقیقات مالاریا بسیار کاربردی هستند؛ می‌توانند در آینده جایگزین روشهای معمول شوند.</div> <div style="text-align: justify;">Background and Aim: To manage and control malaria- one of the most important infectious diseases in tropical and subtropical regions- a quick and accurate malaria diagnosis method is necessary as a high priority. For this purpose, several methods have been developed. Currently, examining the peripheral blood slide with an optical microscope as a gold standard method, and rapid diagnosis method are used in malaria endemic areas. This article reviews the existing methods and the possible future options to improve the diagnosis of malaria. Methods: To prepare the present review article, English language articles with no limitation in publication date were reviewed by searching keywords including malaria diagnosis, microscopic and molecular methods, RDT,Nested- PCR, and LAMP in PubMed and Google Scholar databases. Among the collected articles, the most relevant research articles were selected and reviewed. Conclusion: Despite its limitations, direct observation of peripheral blood smears with a light microscope is still considered the gold standard method for malaria diagnosis. Another malaria diagnosis method is the rapid diagnostic test (RDT), which is a quick and cheap alternative for malaria diagnosis in endemic areas. Regarding malaria elimination programs, the diagnosis of Plasmodium in individuals without symptoms of malaria is challenging as they are generally not detected by routine methods such as peripheral blood smear and rapid diagnostic tests. For this reason, molecular methods are suitable for use in the endemic areas under elimination.
