Malakoplakia is a pseudogranulomatous disorder secondary to an still unknown de - feet in some stages of phagocytosis.  We have reported a 3 year-old girl afflicted with this defect, who was admit­ted to Loghman Hospital' Pediatrics Department with severe pallor, abdominal mass and loonstanding history of dysentery prior to hospitalisation. Inconclusive clinical and B.M. Investigations rendered surgical exploration mandatory on the patient.  Biopsy specimens obtained from appendix and mesenteric lymph nodes excised during operation, revealed extensive histiocytes and neutrophils infiltrate in mucosa! and submucosal of appendix and in lymph nodes, blue tinged Michaelis Gut­mann bodies inside the histiocytes with lack of evidence of malignancy were all in favor on malakoplakia. Urinary infection due to pathogenic E.coli diagnosed during hospitalisation was of interest, owing to the fact that some previously reported cases had similar findings.
