fa بررسی اثر آماده سازی بر پیشگیری از عفونت پس از سپتوپلاستی بین رشته ای ( مدیریت آموزشی، تحقیقات آموزشی، آموزش پزشکی ) Interdisciplinary (Educational Management, Educational research, Statistics, Medical education پژوهشی Original چکیده سابقه و هدف: عمل جراحی سپتوپلاستی از اعمال شایع و پایه‌ای در رشته گوش و حلق و بینی می‌باشد و عفونت در ناحیه بینی به دلیل وجود ارگانیسم‌های متعدد و نزدیکی و ارتباط با مناطق حیاتی سر و گردن از اهمیت ویژه‌ای برخوردار است. تحقیق حاضر با هدف تعیین تاثیر آماده‌سازی (Preparation) قبل از عمل سپتوپلاستی و تأثیر آن در پیشگیری از عفونت پس از عمل انجام گردید. روش بررسی: مطالعه به روش کارآزمایی بالینی بر روی ۱۵۰ نفر ( ۷۸ نفر به عنوان گروه مورد و ۷۲ نفر بعنوان گروه شاهد) انجام گرفت. برای گروه مورد آماده‌سازی قبل از عمل انجام نشد و برای گروه شاهد این کار به صورت روتین صورت گرفت. در نهایت کلیه بیماران از نظر عفونت پس از عمل (روز اول و دوم، هفته اول و دوم، و ماههای یک، دو و سه) تحت بررسی قرار گرفتند. یافته‌ها: گروه مورد شامل ۲۲ زن و ۵۶ مرد با میانگین سنی 1/5±6/24سال و گروه شاهد شامل ۱۷ زن و ۵۵ مرد با میانگین سنی 4/6±1/25 سال بودند. پس از عمل در هر گروه یک مورد عفونت دیده شد. نتیجه‌گیری: با توجه به تحقیق حاضر که حاکی از عدم تأثیر آماده‌سازی ناحیه عمل در میزان بروز عفونت پس از عمل دارد به نظر می‌رسد در اعمال سپتوپلاستی آماده‌سازی دقیق قبل از عمل ضروری نبوده و بهتر است دقت بیشتری در مورد تکنیک عمل، کاهش زمان عمل و مراقبتهای پیش و پس از عمل انجام شود. واژگان کلیدی: سپتوپلاستی، آماده‌سازی، عفونت پس از عمل. Abstract Background: Septoplasty is one of the most common and basic procedures in ear, nose and throat surgery. Due to the presence of several resident organisms and their proximity to vital regions of the head and neck, any infection in this region is of special importance. Implementation of optimal preparation (Prep) of the surgery site has been widely accepted as the most important inhibiting factor for post surgical infection in all branches of surgery including ENT, so we decided to study the effects of preoperative prep on the incidence of post operative infections. Materials and method: This study was conducted as a clinical trial on septoplasty candidates chosen among patients visiting the ENT clinic of Boo Ali and Taleghani hospitals during the years 1997 - 2005. 150 candidates were chosen (78 as cases and 72 as the control group) according to order of visit. Preoperative prep was omitted in all the subjects in the case group, while routine prep with Povidone –Iodine was implemented in controls. Both groups were examined for post surgical infections. Statistical analyses were done by SPSS and Fisher Exact Test. Results: In the case group were 22 females and 56 male with average age of 24.6 ± 5.1 years and in control group were 17 female and 55 male with average 25.3 ± 6.4 years. Signs of infection were observed among one patient in each group (p<0.9). Conclusion: The current study revealed no change in the incidence rate of post-op infection.
