fa بررسی سطح ایمنی علیه ویروس هپاتیت B در دانشجویان ورودی دانشگاه علوم پزشکی شهید بهشتی، سال تحصیلی 92-1391 كاربردي Applicable <p><strong>سابقه و هدف:</strong> واکسیناسیون علیه آنتی ژن سطحی هپاتیت B به عنوان مؤثرترین راه پیشگیری، قابلیت القای سطوح محافظت کننده آنتی بادی ( بیشتر از 10 واحد بین المللی بر لیتر) در بیشتر افراد واکسینه شده را داراست. اما نشان داده شده که تیتر این آنتی بادی به مرور زمان کاهش می یابد. با توجه به نگرانی در مورد سطح ایمنی علیه ویروس هپاتیت B و عوارض شناخته شده آن برای همه به ویژه دانشجویان گروه پزشکی و خلاء اطلاعاتی در مورد وضعیت آن در دانشگاه علوم پزشکی شهید بهشتی این تحقیق در سال تحصیلی 92-91 انجام گرفت.<br> <strong>روش بررسی:</strong> در این مطالعه توصیفی، 441 دانشجوی ورودی سال 1391 دانشگاه علوم پزشکی شهید بهشتی از نظر تیتر HBsAb بررسی شدند. چون همه افراد طبق پروتکل کشوری تحت پوشش واکسیناسیون قرار گرفته بودند، کارت واکسیناسیون از آنها درخواست نگردید. تیتر HBsAb برابر mIU/mL 10 و بالاتر به عنوان سطح محافظت کننده (protective) تعیین گردید. یافته ها به تفکیک جنس نیز بررسی شد.<br> <strong>یافته‌ها</strong>: از 441 فرد مورد مطالعه، 221 نفر (50%) به عنوان افراد ایمن (Immune) شناسایی شدند. میانگین تیتر HBsAb در افراد ایمن mIU/mL 139 و در افراد غیرایمن mIU/mL 9/2 بود (01/0p<). میانگین تیترHBsAb در زنان (mIU/mL 4/80) به طور معنی داری بالاتر از مردان (mIU/mL50) بود (05/0p<). بررسی سطح HBcAb در 135 نفر از افراد مورد مطالعه نشان داد که 1/1% آنان در اثر تماس با ویروس عامل بیماری و بقیه آنان تحت تاثیر واکسیناسیون هپاتیت B واجد آنتی‌بادی گردیده‌اند.<br> <strong>نتیجه‌گیری: </strong>به نظر می‌رسد سطح ایمنی علیه ویروس هپاتیت B پایین است و به ویژه در گروه پزشکی جای نگرانی دارد. لذا بررسی علل آن و انجام اقدامات لازم جهت افزایش سطح ایمنی این گروه از دانشجویان پیشنهاد می‌گردد.</p> <p><strong>Background</strong>: Vaccination against hepatitis B surface antigen is the most effective way for prevention and has the capability to induce protective antibody levels (> 10 IU per liter) for most vaccinated people, but antibody titer has been shown to decrease over time. It is highly important to investigate this subject especially in the medical community and related professions as high-risk groups. Hence, this study was conducted on freshman students in academic year 2012-2013 at Shahid Beheshti University of Medical Sciences, due to concerns about their immunity against hepatitis B and its complications, particularly for medical students and lack of information on its status.<strong><br> Materials and methods: </strong>A total of 441 freshman students were examined for HBsAb titers in this descriptive study in 2012 at Shahid Beheshti University of Medical Sciences. As all students had been vaccinated according to the national protocol, vaccination card was not requested from them. Given the kit used, HBsAb titers &ge;10 mIU/mL was determined as a protective level. The findings were also analyzed according to gender.<br> <strong>Results</strong>: From a total of 441 students, 221 students (50%) were identified as immune people. Mean HBsAb titer was 139 mIU/mL in immune individuals and 2.9 mIU/mL in non-immune people. The difference between mean HBsAb titer in the two groups was found significant using t-test (p<0.01). Mean HBsAb titer was 80.4 mIU/mL in women and 50mIU/mL in men. The findings showed that the antibody titer against hepatitis B surface antigen was significantly higher in women than in men (p<0.05). Also, HBcAb levels in 135 students showed that 1.1% of participants had developed the antibody through contact with the causative agents and the rest of them through hepatitis B vaccination.<br> <strong>Conclusion</strong>: It appears that immunization against hepatitis B virus is low and particularly a concern for the medical team.
