other افتادن دندان های شیری و خطر ورود آنها به درون دستگاه تنفسی و یک مورد پنومونی مزمن The Danger of the Shedding Decidous Teeth Aspiration into the Bronchus and a Case Report of a Chronic Pneumonia in this Regard بین رشته ای ( مدیریت آموزشی، تحقیقات آموزشی، آموزش پزشکی ) Interdisciplinary (Educational Management, Educational research, Statistics, Medical education پژوهشی Original <div style="text-align: justify;">افتادن دندان های شیری کودکان در سنین 6-13 سالگی و خطر ورود آنها به درون نایژه در اثر دستکاری و ندرتا خود به خود به عنوان جسم خارجی در ریه تا کنون آنگونه که باید مورد توجه قرار نگرفته است و چون امکان دارد زمان مکش (آسپیراسیون) دندان نامشخص باشد؛ و از طرف دیگر، به علت تحلیل رفتن ریشه دندان های شیری در پرتونگاری تنها شکل نامنظم تاج دندان های شیری در شش ها به تصویر در می آید. در اثر عفونت های ثانوی این گونه موارد - که از شیوع آنها تاکنون ذکری به میان نیامده - به صورت پنومونی مزمن خود را نشان می دهد و اگر پزشکان حضور ذهن نداشته باشند به تشخیص نمی رسند.ذکر یک مورد پنومونی از بخش عفونی اطفال مرکز پزشکی لبافی نژاد و عنوان کردن شیوع زیادتر آسپیراسیون دندان های شیری لق شده نسبت به آمارهای قبل برای اولین بار است که در نوشتارهای پزشکی مطرح می شود که جا دارد مورد توجه دندانپزشکان و پزشکان قرار گیرد. در بررسی های به عمل آمده در مورد بیماری که به پنومونی مزمن دچار بود، معلوم شد که بیمار حدود یک ماه و نیم قبل به منظور کشیدن دندان فاسد به دندانپزشک مراجعه کرده بود. هنگام عمل دندان آسپیره می شود و با توجه به اینکه علائم حاد قابل توجهی نیز بروز نمی کند، چنین تصور می شود که دندان مورد نظر بلعیده شده است. ولی بعدا بتدریج دچار نارسایی های تنفسی می شود و بالاخره به علت پنومونی مزمن بستری می گردد. موضوع جالب توجه دیگر این که هنگام بیهوشی و برونکوسکپی دندان های دیگری نیز که لق بود، کنده و بلعیده می شود که پس از برونکوسکپی با انجام پرتونگاری این دندان ها در معده مشاهده شدند. </div> The possibility of the shedding primary teeth aspirated into the bronchus is not taken into the consideration sofar as this matter could happen, particularly when it occurs without initial respiratory symptoms, or this episod may be forgotten. Tooth is a nonirritating foreign body and may produce few symptoms even after a prolonged time in the bronchus. Shedding primary teeth usually are only the crowns, their roots are naturally rcsorbed and they are smaller and irregular. The physicians arc not so much familiar with the morphology of the crown of these teeth and their calcification radiologically in the chest film, especially when there is nutritional, developmental and hereditary problems, in which shedding of the primary teeth happen in the younger ages, so; the possibility of the aspiration is hi i;her. Case report; An 8 year old girl, refcred to our pediatric infectious ward as a case of chronic preemonitis, carcfcel observation of the lateral chest film revealed. An opacity in the hilar area similar to the crown of a molar teeth.
