fa بررسی اثر پروبیوتیک لاکتوباسیلوس رامنوسوس بر بیان ژن بتالاکتاماز blaCMY در جدایه‌های بالینی اشریشیا کلی (Escherichia Coli) Evaluation of the Probiotic Effect of Lactobacillus Rhamnosus on blaCMY Beta- Lactamase Gene Expression in Clinical Isolates of Escherichia Coli میکروب‌شناسی Microbiology پژوهشی Original <div style="text-align: justify;">سابقه و هدف: اشرشیا کلی به عنوان یکی از اعضای خانواده انتروباکتریاسه که از گروه باسیل‌های گرم منفی بی‌هوازی اختیاری به شمار می‌رود، می‌تواند از عوامل مهم ایجاد کننده عفونت‌های بیمارستانی در بخش مراقبت‌های ویژه باشد. مقاومت آنتی‌بیوتیکی به دلیل استفاده گسترده از آنتی‌بیوتیک‌ها، یکی از بزرگ‌ترین علل ناکامی در درمان آنتی‌بیوتیکی است. لاکتوباسیلوسها از باکتری‌های استید لاکتیکی(LAB)، از مهم‌ترین سویه‌های پروبیوتیک‌ها به شمار می‌آیند که از نظر خواص ضد سرطانی نیز مورد توجه قرار گرفته‌اند. از آنجا که اثر ضد میکروبی برخی پروبیوتیک‌ها مشخص شده است؛ هدف از این تحقیق بررسی تأثیر پروبیوتیک لاکتوباسیلوس رامنوسوس بر بیان ژن بتالاکتاماز blaCMY در جدایه‌های بالینی اشریشیا کلی است. روش کار: در این مطالعه تجربی، تعداد 60 ایزوله اشرشیا کلی از نمونه‌های بیمارستانی در شهر مشهد و نیشابور جمع‌آوری شد و توسط آزمایش‌های میکروبیولوژی و بیوشیمیایی تأیید شد. آزمایش تعیین حساسیت دارویی نمونه‌ها با روش انتشار از دیسک نسبت به آنتی‌بیوتیک‌ها انجام شد، سپس ایزوله‌های مقاوم جداسازی شدند و پس از کشت دوباره تحت تیمار با عصاره پروبیوتیک قرار گرفته و حداقل غلظت ممانعت‌کننده از رشد (MIC) آنها نسبت به این آنتی‌بیوتیک‌ها توسط روش رقت‌های متوالی طبق استانداردهای CLSI انجام شد. سپس با استفاده از Real- time PCR بیان ژن blaCMY در نمونه‌های کنترل و تیمار بررسی و مقایسه شد. آنالیز آماری با دستگاه SPSS Statistical (27.0.1) IBM (آزمون T مستقل) انجام شد. یافته‌ها: یافته‌ها نشان داد که بیان ژن در نمونه‌های تیمار شده با عصاره پروبیوتیک لاکتوباسیلوس رامنوسوس به طرز معنا‌داری نسبت به نمونه‌های کنترل کاهش یافته است (0/001P < ) . به طوریکه میانگین و انحراف معیار گروه تیمار 0/1 ± 0/35 و  گروه کنترل 0/1 ± 1 است. نتیجه‌گیری: به طور کلی می‌توان نتیجه گرفت احتمالاً عصاره پروبیوتیک لاکتوباسیلوس رامنوسوس از طریق توانایی اتصال، قدرت رقابت و قدرت مهار یوروپاتوژن ها اثر باکتریوسیدی خود را اعمال کرده است.</div> Background and Aim: Escherichia coli, as a member of the Enterobacteriaceae family, belong to the group of facultative anaerobic gram-negative bacilli, and could be an important cause of hospital infections in intensive care units. Due to the widespread use of antibiotics, antibiotic resistance is one of the biggest causes of antibiotic treatment failure. Lactobacillus lactic acid bacteria (LAB) are one of the most important strains of probiotics, which have also been considered for their anti- cancer properties since the antimicrobial effects of some probiotics have been determined. This research aims to investigate the effect of Lactobacillus rhamnosus probiotic on blaCMY beta- lactamase gene expression in Escherichia coli clinical isolates. Methods: In this experimental study, 60 Escherichia coli isolates were collected from hospitals in Mashhad and Neishabur and confirmed by microbiological and biochemical tests. The test to determine the drug sensitivity of the samples to antibiotics was done by the disc diffusion method. Then the resistant isolates were isolated and after re-cultivation, they were treated with probiotic extract. After that, their minimum growth inhibitory concentration (MIC) was compared to these antibiotics by conducting serial dilution method according to CLSI standards. Then, using real- time PCR, the expression of blaCMY gene in control and treatment samples was investigated and compared with each other. Statistical analysis was done with SPSS (independent t-test). Results: The findings showed that the gene expression in samples treated with Lactobacillus rhamnosus probiotic extract was significantly reduced compared to the control samples (P < 0.001). The mean and standard deviation of the treatment group and control group are 0.35 ± 0.1 and 0.1 ± 1 respectively.
