fa مروری بر نقش بیومارکری DNA توموری در گردش (ctDNA) در تشخیص بیماری سرطان A Review of the Role of Circulating Tumor DNA (ctDNA) Biomarkers in Cancer علوم سلولی( سلولی مولکولی، سلول های بنیادی) Cellular Sciences (Molecular Cells, Stem Cells) مروری Review <div style="text-align: justify;">سابقه و هدف: تشخیص زودهنگام از مهم‌ترین عوامل در پاسخ به درمان در بیماران مبتلا به سرطان است. تشخیص سرطان در مراحل اولیه نیازمند کشف بیومارکرهای حساس و اختصاصی است. این بیومارکرها می‌توانند شامل پروتئین‌ها (انواع آنزیم‌ها، گیرنده‌ها، آنتی‌بادی‌ها و پپتیدها)، اسیدهای نوکلئیک )از جنس DNA، مانند DNA بدون سلول (cfDNA) و یا از جنس RNA، مانندmicroRNA  یا سایرRNA های غیر کدکننده( باشد. هدف از انجام این تحقیق، بررسی نقش بیومارکری DNA توموری در گردش (ctDNA) در تشخیص بیماری سرطان است. روش کار: برای دستیابی به مقاله‌های مرتبط با هدف تحقیق، کلیدواژه‌های ctDNA، cancer و diagnosis در بانک‌های اطلاعاتی Google Scholar، PubMed و Web of Science جست‌وجو شد. پس از بررسی و مرور 312 مقاله یافت‌شده، مقاله‌های فاقد ارتباط موضوعی دقیق با عنوان و هدف پژوهش از مطالعه خارج شده و در نهایت، 174 مورد از مقاله‌های مرتبط وارد مطالعه شدند. پس از مرتبط‌سازی یافته‌ها، مقاله‌ها مطالعه شدند و محتوای مورد نظر اقتباس شد. یافته‌ها: به نظر می‌رسد کهcfDNA ، حاصل تکه تکه شدن <st1:stockticker w:st="on">DNA</st1:stockticker> در نتیجه‌ی آپوپتوز، نکروز، ترشح فعال، آسیب یا مرگ سلول و بافت‌های مختلف است که به داخل جریان خون آزاد می‌شوند. cfDNA نیز به عنوان یک بیومارکر برای تشخیص، بررسی عود و پیش‌آگهی برخی سرطان‌ها استفاده می‌شود. همچنین، ctDNA نیز جزئی از cfDNA است که شامل قطعه‌های کوچکی از <st1:stockticker w:st="on">DNA</st1:stockticker> با طول کمتر از 200 نوکلئوتید است. ctDNA از سلول‌های توموری درحال گردش در جریان خون و سیستم لنفاوی مشتق می‌شود. همچنین ممکن است ctDNA مستقیماً از سلول‌های محل تومور که در اثر آپوپتوز و نکروز در حال مرگ‌اند و یا از طریق رهاسازی فعال از سلول‌های سالم توموری در جریان خون و سیستم لنفاوی آزاد شوند. مقدار ctDNA در افراد مختلف، متفاوت است و به نوع تومور، محل آن و برای تومورهای سرطانی، به مرحله سرطان بستگی دارد. ارزیابی میزان ctDNA، ردیابی جهش در ctDNA و بررسی رخداد متیلاسیون نابجای ctDNA از بیومارکرهای مطرح در تشخیص، تعیین پیش‌آگهی و تعیین مسیر درمان برای بیماران سرطانی است. کنترل سرطان‌ها با اندازهگیری ctDNA دینامیک در خون، پلاسما یا سرم، یک بخش جدید و در حال توسعه در تحقیق‌هاست. نتیجه‌گیری: به نظر می‌رسد که با وجود چالش‌های موجود در کاربرد بالینی ctDNA ها، آنها می‌توانند در تشخیص زودهنگام سرطان استفاده شوند.</div> Background and Aim: Early diagnosis is one of the most important factors in cancer patients’ treatment. Early detection of cancer requires the discovery of sensitive and specific biomarkers. These biomarkers include proteins (i.e., enzymes, receptors, antibodies and peptides), nucleic acids (DNA- based, such as cell-free DNA (cfDNA) or could be RNA-based, such as microRNA or other non-coding RNAs).  In this review, we investigated the role of circulating tumor DNA (ctDNA) biomarker in the diagnosis of cancer. Methods: In order to find articles related to the purpose of the research, keywords ctDNA and cancer and diagnosis were searched in Google Scholar, PubMed and Web of Science databases. After reviewing 312 found articles, the articles without exact subject and purpose relevance were excluded from the study. Finally, 174 related articles were included in the study. After linking the findings, the articles were read and the desired content was adapted. Results: It seems that cfDNA is the result of DNA fragmentation after apoptosis, necrosis, active secretion and damage or death of cells and different tissues and is released into the bloodstream. cfDNA is also used as a biomarker for the diagnosis, recurrence and prognosis of some cancers. Also, ctDNA is a part of cfDNA, which includes small pieces of DNA with a length of less than 200 nucleotides. ctDNA is derived from tumor cells circulating in the bloodstream and lymphatic system. Also, ctDNA may be released directly from cells in the tumor site that are dying due to apoptosis and necrosis, or through active release from healthy tumor cells in the bloodstream and lymphatic system. The amount of ctDNA in different people is different and depends on the type of tumor, its location and for cancerous tumors, the stage of the cancer. Assessing the amount of ctDNA, tracking its mutations and investigating the occurrence of aberrant methylation of ctDNA are important biomarkers in diagnosis, determining prognosis, and the course of treatment for cancer patients. Controlling cancers by measuring dynamic ctDNA in blood, plasma or serum is a new and developing area of research.
