fa ارتباط پلی‌مورفیسم-1486T/C ژن TLR9 با خطر ابتلا به زخم پپتیک و عفونت هلیکوباکترپیلوری Association of the TLR9 -1486T/C Gene Polymorphism with the Risk of Peptic Ulcer Disease and Helicobacter pylori Infection ژنتیک Genetic پژوهشی Original <div style="text-align: justify;">سابقه و هدف: زخم پپتیک سبب بروز زخم‌های باز در پوشش معده یا دوازدهه شده و عفونت هلیکوباکترپیلوری یک عامل اصلی بروز آن است. TLRها در تشخیص عفونت هلیکوباکترپیلوری، تنظیم و فعال‌سازی سیستم ایمنی میزبان علیه آن نقش مهمی دارند.TLR-9 ، با شناسایی DNA باکتری‌های مهاجم به سلول‌های اپی‌تلیالی میزبان، مسیر سیگنالینگ سیستم ایمنی را فعال می‌کنند. هدف از این مطالعه بررسی ارتباط بین پلی‌مورفیسم -1486T/C ژن TLR9 با خطر ابتلا به زخم پپتیک و عفونت هلیکوباکترپیلوری بود. روش کار: در این مطالعه موردی- شاهدی، 96 بیمار مبتلا زخم پپتیک دارای عفونت به هلیکوباکترپیلوری و 104 فرد سالم بررسی شدند. فراوانی ژنوتیپی پلی‌مورفیسم -1486T/C ژن TLR9 با روش PCR-RFLP  تعیین و داده‌ها با  آزمون مجذور کای دو آنالیز شدند. یافته‌ها: نتایج آنالیز آماری نشان داد، اختلاف آماری معناداری در فراوانی ژنوتیپی CC (0/001 = P، 8/76-1/62= CI، 3/77 =OR ) و  فراوانی آللی C (0/0002 = P، 3/23-1/41= CI، 2/13 = OR) بین بیماران مبتلا به زخم پپتیک همراه با عفونت هلیکوباکترپیلوری و کنترل مشاهده شد. نتیجه‌گیری: به نظر می‌رسد حضور آلل C در جایگاه پلی مورف -1486T/C ژن TLR9 با خطر ابتلا به زخم پپتیک و عفونت هلیکوباکتری در جمعیت مطالعه شده مرتبط باشد. مطالعه‌های همگروهی و یا تجربی را توصیه می‌کنیم.  </div> <div style="text-align: justify;">Background and Aim: Peptic ulcer disease (PUD) is characterized by open sores in the lining of the stomach or duodenum, and Helicobacter pylori infection is a major etiological factor. Toll- like receptors (TLRs) plays an important role in detecting H. pylori infection and regulating and activating the host immune response. TLR9 activates immune signaling pathways by recognizing bacterial DNA invading host epithelial cells. The aim of this study was to investigate the association between the TLR9 gene -1486T/C polymorphism and the risk of PUD and H. pylori infection. Methods: In this case- control study, 96 patients with PUD and H. pylori infection and 104 healthy individuals were enrolled. The genotypic frequency of the -1486T/C polymorphism of the TLR9 gene was determined using the PCR- RFLP method, and data were analyzed by the chi-square test. Results: Statistical analysis showed a significant difference in the frequency of the CC genotype (P = 0.001, CI = 1.62-8.76, OR = 3.77) and the C allele (P = 0.0002, CI = 1.41-3.23, OR = 2.13) between patients with PUD and H. pylori infection and the control group. Conclusion: The presence of the C allele at the -1486T/C polymorphism of the TLR9 gene appears to be associated with an increased risk of peptic ulcer disease and H. pylori infection in the studied population.
