fa شیوع پنومونی پنوموسیستیس (PCP)، هپاتیت و سل در بیماران HIV مثبت و عوامل مرتبط با آن: مطالعه‌ای با تأکید بر نقش تعیین‌کننده سطح CD4 Prevalence of HIV Comorbidities and Their Association with CD4+ Cell Counts among HIV- Positive Individuals Covered by Khorramabad Counseling Centers (2010-2023) ویروس شناسی Virology پژوهشی Original <div style="text-align: justify;">سابقه و هدف: عفونت‌های فرصت‌طلب و همراه، یکی از مهم‌ترین علل مرگ‌ومیر و کاهش کیفیت زندگی در بیماران HIV مثبت محسوب می‌شوند. این مطالعه با هدف تعیین شیوع عفونت‌های مهم همراه بیماری HIV و بررسی عوامل مرتبط با آنها، با تأکید ویژه بر رابطه سطح سلولهای CD4+ T-، در جمعیت بیماران تحت پوشش مراکز مشاوره خرم‌آباد طراحی شد. روش کار: در این مطالعه مقطعی- تحلیلی، پرونده‌های پزشکی ۳۰۰ بیمار HIV مثبت مراجعه‌کننده به مراکز مشاوره بیماری‌های رفتاری خرم‌آباد در بازه زمانی ۱۳۹۰ تا ۱۴۰۲ بررسی شد. در این بررسی، پایین‌ترین سطح ثبت‌شده سلول‌های CD4+ T- و نیز آخرین شمارش آن در هر بیمار استخراج و تحلیل شد. داده‌ها با استفاده از نرم‌افزار SPSS نسخه ۲۴ و با به کارگیری آزمون‌های آماری مجذور کای و تست دقیق فیشر برای بررسی ارتباط بین متغیرهای کیفی و سطح سلولهای CD4+ T- تحلیل شد. سطح معناداری (0/05p < ) در نظر گرفته شد. یافته‌ها: بررسی نتایج نشان داد که 36/7درصد مبتلا به PCP، 10/7 درصد دارای HCVAb، 4 درصد دارای سل آشکار و 4/7 درصد دارای تست PPD با پاسخ 5 سانتیمتر یا بزرگتر، 1 درصد دارای Ab Toxo IgG مثبت بودند. بین تعداد سلولهای CD4 با ابتلا به PCP ارتباط معناداری وجود داشت (0/001p < ). اما با ابتلا به سل، هپاتیت B، هپاتیت C و توکسوپلاسما ارتباط معناداری مشاهده نشد. نتیجه‌گیری: به نظر میرسد که سطوح بسیار پایین سلولهای CD4+ T- با افزایش خطر ابتلا به PCP مرتبط است. این نتایج بر اهمیت پروفیلاکسی و غربالگری هدفمند برای PCP در بیماران با نقص ایمنی شدید تأکید می‌کند.  </div> <div style="text-align: justify;">Background and Aim: Opportunistic and concomitant infections are among the most important causes of mortality and reduced quality of life in HIV- positive patients. This study was designed to determine the prevalence of major concomitant infections in patients with HIV and to investigate their associated factors, with special emphasis on the relationship with CD4 lymphocyte levels, among patients covered by counseling centers in Khorramabad. Methods: In this cross- sectional analytical study, the medical records of 300 HIV- positive patients referred to behavioral disease counseling centers in Khorramabad between 2011 and 2023 were reviewed. The lowest recorded CD4+ T-cell level and the most recent CD4 count for each patient were extracted and analyzed. Data were analyzed using SPSS software version 24. The Chi-square test and Fisher’s exact test were used to assess the relationship between qualitative variables and CD4 levels. A significance level of P < 0.05 was considered statistically significant. Results: The findings showed that 36.7% of patients had Pneumocystis pneumonia (PCP), 10.7% were positive for HCV antibodies (HCVAb), 4% had overt tuberculosis, 4.7% had a PPD test response of 5 mm or greater, and 1% were positive for Toxoplasma IgG antibodies. A significant association was observed between CD4 cell count and PCP (P < 0.001). However, no significant relationship was found between CD4 count and tuberculosis, hepatitis B or C, or toxoplasmosis. Conclusion: In the studied population, very low CD4 levels were associated with an increased risk of developing PCP.
