Abstract Background: Radial head fracture is the most common bony injury of the elbow. Previous surgical approaches by severing the Orbicularis ligament were associated with instability. Wrightington approach is a new method in which the ligament remains intact with better results. Current study was performed to compare the results of surgical fixation of the radial head fracture with Wrightington and Kocher approaches. Materials and methods: In this clinical trial 60 patients with radial head fracture were divided equally into Wrightington and Kocher groups outcomes were assessed one year postoperatively patients were examined for range of flexion/ extension, supination and pronation, presence of pain and any complications, the results were documented and the two groups compared statistically. Results: Range of flexion/extension was significantly greater in the Wrightington group, (142±25 degrees) vs. 125±29 degrees in the Kocher group, (P<0.05). There was no statistically significant difference between the groups in the terms of range of supination and pronation and presence of pain. There was no patient with infection. In total, 16.7% of patients in Kocher group and 6.7% of patients in the Wrightington group suffered from limited range of motion. In addition, PIN was injured in 3.3% of patients in the Kocher group. However, there was no statistically significant difference between the two groups in complications. Conclusion: Because of the intact lateral ligamentous complex and less injury to soft tissue it seems Wrightington approach is superior to Kocher approach in the treatment of patients with radial head fracture.
