Abstract Background: Consumption of substance, alcohol and cigarettes in candidates for transplant surgery is a major problem as these habits can decrease the rate of success in transplant surgery. Since Masih Daneshvari Hospital is a center for heart and lung transplant this study was done to gather information about the prevalence of cigar and substance use among patients on the waiting list for transplant surgery. Materials and methods: In this cross-sectional study, Addiction Severity Index (ASI) was completed by 66 patients on heart and lung transplant waiting lists. Results: All patients had history of cigarette smoking 71% patients were smokers at the time of study and 79% of patients had another smoker member in their families. The daily use of cigarette among 77% of patients, who smoked at the time of this study, was less than 20 cigarettes per day. Over 80% of patients had no information about the adverse effects of cigarette smoking. 76% of patients believed that they would quit the habit in the next 5 years 29% had a history of substance use, inhalational opium being the most common substance that was used. Conclusion: All patients in our study had history of cigarette smoking and about one third of patients had history of substance or alcohol use and more than two third of them continued smoking. Most of the patients had little information or no information about hazards of tobacco smoke or its consequent mortality rate. Educational intervention for prevention and management of substance abuse and cigarette smoking is crucial for candidates of transplant surgery.
