Background and Aim: Peptic ulcer disease (PUD) is characterized by open sores in the lining of the stomach or duodenum, and Helicobacter pylori infection is a major etiological factor. Toll- like receptors (TLRs) plays an important role in detecting H. pylori infection and regulating and activating the host immune response. TLR9 activates immune signaling pathways by recognizing bacterial DNA invading host epithelial cells. The aim of this study was to investigate the association between the TLR9 gene -1486T/C polymorphism and the risk of PUD and H. pylori infection.
Methods: In this case- control study, 96 patients with PUD and H. pylori infection and 104 healthy individuals were enrolled. The genotypic frequency of the -1486T/C polymorphism of the TLR9 gene was determined using the
PCR- RFLP method, and data were analyzed by the chi-square test.

Results: Statistical analysis showed a significant difference in the frequency of the CC genotype (P = 0.001,
CI = 1.62-8.76, OR = 3.77) and the C allele (P = 0.0002, CI = 1.41-3.23, OR = 2.13) between patients with PUD and H. pylori infection and the control group.

Conclusion: The presence of the C allele at the -1486T/C polymorphism of the TLR9 gene appears to be associated with an increased risk of peptic ulcer disease and H. pylori infection in the studied population. However, further studies with larger and more diverse populations are needed to confirm these findings.
