Serios otitis media is one of those ear Trou­bles that has enjoyed a very extensive research two decaides. 
I ts recognition in medical literature goes b,ick tc eighteenth century. Since then there has been rather little progress in this field. The dis­covery of antibiotcs and thence lessening of in­
fecticus ear diseases, attention has been paid to preserve the function of ear, and naturally light has been directed to the problem of ear effusion vnd th;s ill part has been respnnsible for detect­ing and curing this ailment successfully and 
efficiently, and to scme extent there has been a rather good progress in understanding its ethio­logy and pathology. 
In th;s article with regard to aforemention­ed facts the subject has been discussed in the following manner. 
1- Eth;ology: Allergy and upper respira­tory tract, infections, misuse of antibiotics and Eustachian tube insufficiency h􀊸s been display­ed. 
2- HistopAthologv: In this dise!:lse the mu­cos3l linning of the middle ear cleft is changed and there :s methaplask changes from single squ;:imous ep,ithelium to respiratory type of epi-thelium, that is pseudostratifed ciliated columnar epithelium with secretory goblet cells. 
3- Diagnosis: this is based on three factors: a- hearing impairment stated by patientor his parents, and appearance of tympanic mem­brane. 
b- audiometric and impedance measure­ment. 
c- paracenthesis and drainge of the middle ear cleft fluid. 
4- M:a1pgcment, is as follows:
a- Treatment of predispesing factors l;keallergy, adenoids. sinusitis and etc. 
b- fluid drainge after parncenthes•s.
c- Teflen Grommet Drain tube insertion.
At the conclusion a few intresting cases have been discussed. which have been seen by the writer.
